Acute Hemorrhoid Symptoms — Diagnosis, Treatment, Doctor Visit

More than half of all adults over 30 experience hemorrhoidal disease at some point — yet many postpone a doctor's visit out of embarrassment. That is a mistake for two reasons: first, early symptoms are usually easy to treat with gentle measures; second, symptoms such as blood in the stool can also indicate other, more serious conditions (anal fissure, perianal thrombosis, colorectal polyps, and more rarely colorectal cancer) that can only be reliably ruled out by medical examination.

Munich has excellent specialist proctology practices and hospital departments, but waiting times are often long and walking into an unfamiliar practice is difficult for many. For a first, discreet assessment and conservative treatment, a private physician house call is an uncomplicated alternative — the physical examination is brief and feels less burdensome in familiar surroundings.

This guide explains how to interpret symptoms, when medical help is appropriate, what a doctor can do during a house call, and when a procedure by a proctologist or surgeon becomes necessary. It is based on current international coloproctology recommendations and the German DGK S3 guideline.

What a doctor can do during the house call

A private physician house call is well suited for a first assessment because the time pressure is lower than in an emergency department and examination in familiar surroundings is more comfortable for many patients. The following services are possible:

Not possible during a house call: proctoscopy, rectoscopy, colonoscopy, rubber-band ligation, sclerotherapy or surgical procedures — these remain the domain of the proctologist or hospital. In selected cases of acute perianal thrombosis within the first 48 hours an outpatient incision by a surgeon may be indicated; the house-call physician arranges the rapid referral.

Why a house call — and why sometimes not?

Hemorrhoidal complaints are a textbook case where a private house call is the lowest-threshold and fastest option:

A house call is not appropriate for heavy acute bleeding with circulatory compromise (call 112 or go to an emergency department), for clearly surgical stage IV disease (go directly to proctology or surgery), or when a colonoscopy is needed for tumour work-up (this requires outpatient or inpatient endoscopy at a Munich practice or hospital). Video consultations are of limited use for anal conditions: the digital rectal examination and visual inspection are the decisive diagnostic steps and cannot be replaced on camera.

How do I recognise hemorrhoids?

Typical signs are bright red blood on the toilet paper or stool, itching and moisture around the anus, a foreign-body sensation, painful lumps near the anus (especially in perianal thrombosis) and a sensation of incomplete emptying. From stage II onwards the cushions prolapse on straining and either retract spontaneously or require manual reduction.

When should I see a doctor for hemorrhoids?

Whenever you see blood in the stool — hemorrhoids are the most common but not the only cause. A medical examination is important also to rule out colorectal cancer or an anal fissure. Further reasons: severe pain, black tarry stool (melaena), fever, unintended weight loss, new bowel changes after age 40, or signs of anaemia.

What can a doctor do during a house call for hemorrhoids?

A private physician inspects and palpates the anal area (digital rectal examination), assesses stage and accompanying findings (thrombosis, fissure, eczema), issues private prescriptions for ointments (lidocaine-containing, steroid) and sitz baths, and provides a targeted referral to a Munich proctologist. For an acutely thrombosed perianal haematoma within the first 48 hours an outpatient incision may be considered.

Do I need surgery for hemorrhoids?

In most cases no. Stage I and II can be treated conservatively (diet, hygiene, ointments). Stage II–III respond to minimally invasive procedures such as sclerotherapy or rubber-band ligation. Only stage III/IV with substantial symptoms or complications usually needs surgery (hemorrhoidectomy, stapled procedure, HAL/RAR). A proctologist decides after a full examination.

Billing follows the GOÄ (German medical fee schedule for private medicine), from 180 € depending on time of day, day of the week and services rendered. Private insurance usually reimburses in full; statutory (public) insurance does not cover this private service.
